To begin your application, please complete the following steps:
  • Fill out our online application.
  • Applicants must call the Staff Providers Main Office to setup an appointment. You will be advised to bring the following documentation:
    • Valid State ID or Driver's License
    • Proof of work authorization (Immigrant Card or US Passport)
    • Copy of recent criminal background check (not older than 6 months)
    • Copy of T.B. test
    • CPR card
    • Certificate of completion of 8-hour In-home personnel service worker orientation training
  • Complete health and background check disclosure and authorization
